Download a subject access form

There are two different forms to use depending on whether the information you want is held nationally or locally.

Nationally held information - Form 1

Download form 1 (nationally held information) if the information you require is from the Police National Computer and relates to your prosecution or conviction history.

Locally held information - Form 2

Download form 2 (locally held information) if the information you require is held locally on Kent Police systems (whether they are computerised or paper records).

NB You must specify clearly what information you want to find out.

 

You can make a subject access request using both forms if you want the search to cover both local and national records. The fee of £10 covers both.
