I'd like to request disclosures for family court proceedings on behalf of my client/local authority
Thank you. To apply, follow these steps:
Step 1: Download the application form (Annex 1).
Step 2: In private law proceedings, you need to make an initial payment using BACS before submitting your application form (Annex 1). You will find the BACS details and guidance within the form. Note that split payments cannot be accepted. Failure to make an initial payment will result in your application being rejected.
In public law proceedings, there is currently no charge.
Step 3: Download and follow either the:
Step 4: Complete the downloaded application form (Annex 1) on a computer, using the relevant guidance notes to help you. Please ensure all mandatory fields are completed otherwise your application will be rejected.
Step 5: Email your completed application form (Annex 1) to our Public Disclosure team, Civil Court Order Section.
In private law proceedings, you must also attach a court approved and sealed 'Private Law Police Disclosure Order (Annex 5)’ with your application form (Annex 1). Failure to do so will result in the application being rejected.
